Our Practice Model
HOW WE OPERATE
The primary care aspect of our clinic will be a shared care model.
What does that mean?
It means you will have a primary provider and you will be invited to book with any of our clinic providers. This model allows for more flexibility for both patients and providers. For example, if your primary provider is not available on your preferred day you can book with another provider who does have availability on your preferred day.
The primary care aspect of our clinic will be multidisciplinary.
What does that mean?
It means that you may be treated by a physician, a nurse practitioner, a physician assistant, and/or one of our advanced medical learners. We are excited about this diversity in training backgrounds as we strongly believe it will make for a more resilient team of medical experts to provide care in our communities.
What does that mean?
Our clinic host learners.
We are excited to incorporate learners (from medical schools, medical residency programs, nurse practitioner programs, and physician assistant programs) into our work and our communities. We believe our model of care is a robust environment for learners to take part in. We are excited to share with them what it is we love about our communities and our work here. We encourage patients to extend that same warm welcome to our students whether you see them in the clinic or out in the community.
Clinic Policies

Appointments are booked at 10 and 20 minute intervals depending on the type of booking. Please be mindful of the time allotted and book separate appointments for multiple issues.

PSH+W will accept same day/walk- in appointments daily to patients of PSH+W and the general public. Availability will be based on demand and will vary each day. Please keep in mind that walk-in visits are intended to be for a single, urgent issue that requires timely care.

PSH+W strives to provide quality care and ensure appointment times are on schedule to the best of our ability. Urgent or emergent matters may arise, and should this happen, understand we are doing our best to accommodate all patients.

Upon being referred to a specialist or for further testing, you will receive a letter from PSH+W. This letter will include who you have been referred to and where, type of speciality/test and the specialists or testing sites contact information. Consult or testing wait times can vary, most taking months.

When requiring medication refills, please allow the clinic to have 1-2 weeks notice. Call your pharmacy to request a refill list sent to PSH+W. If you require an appointment in person prior to receiving further refills, ensure you call the clinic 2-4 weeks in advance. This may include narcotic/triplicate medications.

Certain services such as driver’s medicals, disability forms and travel vaccines are subject to a fee. A list of uninsured services can be found posted in the clinic.

We have a zero-tolerance policy towards all types of abuse. Those who are abusive, will be asked to leave and may be dismissed from the practice permanently.
It is difficult to answer every phone call during a busy clinic day, so please leave ONE non-urgent message and you will be contacted as soon as possible.
